We provide the services of a trained care worker to carry out those tasks that will be enable you to continue managing at home. We also ensure that the careworker/s receive adequate support and ongoing training, with review for client satisfaction and monitoring of working relationship. Our full range of services, in and around the home, such as cleaning and laundry tasks, is not limited to routine housework. This may extend to light maintenance work and or gardening. The tasks will be documented in a task plan. You are responsible for providing all the cleaning agents and equipment for household management tasks. Other duties include personal care assistance - defined as assistance with personal hygiene, grooming, toileting, dressing, feeding, transfers and mobility. Visits to doctors and/or shopping are also provided.

A full subsidy is granted if you hold a Community Service Card. This means that all the costs for your care are covered, but will not necessarily cover your visits to hospitals/specialists outside the South Auckland areas.

The areas covered by our Services, include The Panmure/Glen Innes areas, Mt Wellington, Onehunga, Otara, Papatoetoe, Mangere and Otahuhu, Manurewa, Manukau and Wiri, Takanini and the Papakura districts.

You will be assessed by the NASC team (Needs Assessor and Service Co-ordinator). They are responsible to the MOH for the management of the budget for Disability Community Support Services, and will determine your eligibility for provision of services. They will also decide on the amount and type of services you require, and allow you to choose the Service Provider, that will best meet your assessed needs, so that you can continue to live at home. There is a means and asset test criteria. Generally your Community Service Card, provides for a full subsidy to the services. The needs assessment is completed by a health professional in the presence of family/whanau, to determine your needs. We will ensure that your assessment and service provisions incorporate your special culture and language, and choose the support worker accordingly.

The contract with the MOH/DHB Healthwise requires that quality service standards be upheld; and monitored, through a yearly independent audit review:
* To provide assurance that the standards as agreed in the purchase agreement are being met
* To ensure direction and measure practice against established standards—(NZS 8158:2003).
* A review to action achievements or improvements

Ko te ratonga mo te Hunga Haua – Attendant Care Services – which is also being provided under Home Based Support Services.
Attendant Care is personal care support provided to any person with a degree of physical disability, so that he/she can engage in the activities, pursuits and processes of everyday life. The assessment of attendant care will identify the level and nature of assistance required.
The support they require is for a range of personal care tasks, such as showering, toileting and dressing, and can include, transfers from bed to wheelchair or commode.
The meal preparation, cooking and general housework is provided for in the form of ‘homehelp’ entitlement.
